Athletic Forms
If a student wants to come out for one of our sports, he/she must turn in the Athletic Clearance Packet before participating in tryouts, practices, or contests.
The packet includes:
- Letter from Kevin Kovacs, Athletic Director
- Packet Checklist
- Athletic Clearance form (.PDF 15 KB)
- Annual Physical form completed & signed by a physician
- Medical Authorization Form
- Athlete's Agreement form (.PDF 29 KB)
