Athletic Forms

If a student wants to come out for one of our sports, he/she must turn in the Athletic Clearance Packet before participating in tryouts, practices, or contests.
The packet includes:

1. Letter from Kevin Kovacs, Athletic Director
2. 2010-2011 Packet Checklist
3. Athletic Clearance form (.PDF 15 KB)
4. Annual Physical form completed & signed by a physician
5. Medical Authorization Form
6. Athlete's Agreement form (.PDF 29 KB)
